Please note that the State of Ohio has released an updated version of the Application for Certificate of Title document with a 12/25 revision date. The new form number is BMV-3774(1P)12/25. It replaces the BMV-3774(1P)8/24.
Application for Dealer Assignment
Please note that the State of Ohio has released an updated version of the Application for Certificate of Title document with a 12/25 revision date. The new form number is BMV-3772(3P)12/25. It replaces the BMV-3772(3P)8/24.
Dealers will need to consult with their DMS provider regarding reprogramming requirements for new or updated documents.
