Application for Certificate of Title for a Vehicle

Please note that the State of Oklahoma has released an updated version of the Application for Certificate of Title for a Vehicle document with an 7/25 revision date. The new form number is 701-6(1P)7/25. It replaces the 701-6(1P)5/24.

Dealers will need to consult with their DMS provider regarding reprogramming requirements for new or updated documents.
