WI Title and License Plate Application
Please note that the Wisconsin Department of Transportation DMV has released an updated version of the WI Title and License Plate Application form with a 10/25A revision date. The new form number is MV-11(3P)10/25A. It replaces the MV-11(3P)10/24.
Dealers will need to consult with their DMS provider regarding reprogramming requirements for new or updated documents.
