WI Title and License Plate Application
Please note that the Wisconsin Department of Transportation has issued a revised WI Title & License Plate Application form after an initial delay. The form has a 10/25 revision date, and Dealers can now start using it. The form was modified to reflect the passage of ACT 181 giving the DMV authority to collect a $6.00 per plate issuance fee for new registration plates, effective April 13, 2026.
The new form number is MV-11(3P)10/25. It replaces the MV-11(3P)10/25A.
Dealers will need to consult with their DMS provider regarding programming requirements for new or updated documents.
