Wisconsin Title and License Plate Application
The new form number is MV-11(3P) 10/26. It replaces the MV-11 (3P) 10/25.
Dealers should begin using the new version on October 1. Existing stock can be used until forms are exhausted.
Dealers will need to consult with their DMS provider regarding programming requirements for new or updated documents.
